Description
The Kay and Al Abiouness Advance Care Planning Seminar is a free educational event designed to prepare and guide individuals or families to engage in essential end-of-life care discussions. The Lake Taylor Transitional Care Hospital Foundation proudly presents this empowering seminar.
Immediately following the expert speakers, attendees may visit vendor displays or hold informal discussions with representatives of local, aligned healthcare organizations. All attendees also have the opportunity to create their own Advance Care Plan before they leave the event, confidentially and at no cost, assisted by trained volunteer facilitators.
Advance Care Planning ensures that families, loved ones, or trusted friends are prepared to make decisions when someone they care for can no longer speak for themselves. Often termed "The Courageous Conversation," this seminar aims to make the subject more approachable and the discussions less daunting.

Emily A. Martin, Attorney at Hook Law Center, specializing in elder law, estate planning, and long-term care planning.
